Health Services in the Elkhorn Area School District work to advance the well-being, academic success, and life-long achievement of students. Health Services facilitate positive student responses to normal development, promote health safety, intervene with actual and potential health problems, provide case management services, and collaborate with school faculty and staff to build student and family capacity for adaptation, self-management, self-advocacy, and life-long learning.

**Recommended at the 4th grade level. Middle School and High School Physicals may be required for athletic participation.

***This form also provides families the option to waive required immunizations for religious or personal conviction reasons. Students out of compliance with State immunization requirements may be excluded from school in the event of an outbreak.

Medication forms must be filled out annually for students taking prescription and non-prescription drugs at school.
